Minnesota has a service called Long-Term Care Consultation (LTCC) that provides help for people who don’t know what they need. Some counties may refer to this as MnChoices. LTCC services are provided locally generally based on the county you live in and are free.
What will I get from an assessment? A nurse or social worker will visit your home and discuss your needs and help plan for services. Assessors provide Information about long-term care options and public programs that can also help pay for needed services. There is transition assistance for people who want to return to community settings after admission to a nursing facility.
The LTCC team will make recommendations for community services or nursing facility admission when community services are not feasible; the person makes the final decision about services.
